SMS & communications consent
The consent language patients review and acknowledge in person at check-in before receiving text or email communications from our office.

This page is provided for reference only. Horizon OMS of Chicago obtains SMS and electronic communications consent from patients verbally or in writing during check-in at our office — this page is not a form and does not collect, submit, or store any information. It reflects the consent language patients review and acknowledge in person.
Authorization
By providing consent at check-in, a patient authorizes Horizon OMS of Chicago to communicate with them via text message and/or email.
Types of Communications
These communications may include, but are not limited to:
- Appointment reminders and confirmations
- Office hour updates and scheduling changes
- Billing notifications
- Requests for information related to online patient registration, referral details, or insurance verification
- Other important updates related to the patient's care
Consent Choices
At check-in, patients choose how they would like to receive electronic communications from Horizon OMS of Chicago:
- Text message
- Both text message and email
Patients may also decline electronic communications altogether and indicate that they prefer phone calls for appointment-related information.
Message & Data Rates
Standard message and data rates from the patient's mobile carrier may apply. Message frequency will vary.
Opt-Out Instructions
Patients who have consented to electronic communications can opt out at any time by:
- Replying STOP to any text message from Horizon OMS of Chicago
- Contacting our office directly by phone or in person
